Administrative Autopsy
 
1.  Time for Inspection: Please call to schedule an appointment with the Chief of the Health Station.

2.  Issuance of Death Certificate:

(1)  Required Documents: ID Card of the Deceased

(2)  Issuance Procedures: Register (Registration Counter) → Issue Death Certificate (Diagnosis Room) → Pay and Stamp (Registration Counter)

Note: Administrative Fee of NT$400 (NT$20 for Each Copy of Death Certificate).
